We recognize the special needs of our Seniors. Families and caregivers can get overwhelmed by the complexity of care required for a loved one. Our goal is to support quality of life and a safe environment for our clients. We can provide a range of services from a single consultation with families to explore options to comprehensive care planning.
Some of the services we can provide to you include:
- Exploring housing options and available resources
- Home Safety Assessment and recommendations to improve safety
- Referral to home health service providers, if needed
- Comprehensive Care Management services
- Medication coordination
We are sensitive to family concerns about their loved one's mental health and/or alcohol and drug use. We address these concerns in a dignified, respectful, and confidential way. Our approach consists of the following:
- Senior specific assessment instruments to identify mental health or substance abuse problems that can be completed in a home visit or in our office.
- Intervention services for families who would like their loved one to get help for a mental health or addiction problem.
- Direct referral to programs with specialized mental health and substance abuse services to meet the needs of seniors in the public and private sectors.
- Referral to therapists with expertise treating the elderly.
- Coordination of care with other treatment professionals.
- Monitoring of follow-up recommendations.
- Follow up with family members to review progress.
