Care Management
We at Medical Advocates of Virginia (MAV) have many years of experience providing case management services to adults with complex medical needs. Patients and families can easily become frenzied by medical problems that arise and the often complex choices for treatment of those problems. Multiple medical providers, multiple illnesses, and multiple medications can bewilder the most dedicated caregivers.
Often patients and family members cannot understand the medical terms being used for treatment, making it difficult to choose the appropriate course of action to follow.
In addition, there may be no coordination of care among the primary caregivers. This often leaves the family to frantically try to reach several doctors, monitor several medications, fill out numerous forms, and just plain GUESS what they should do!!
Our Strategy
We recognize that every client and every situation is different. We will design an individualized approach to address the problems, taking into consideration all the relevant information about your loved one.
We use a systematic strategy:
- Assess your individual needs
- Create a feasible plan
- Monitor the progress
- Change the plan as needed
Some of the services we provide include:
- Consult with you to review your needs. We can visit your home to discuss how we can help or you can make an office appointment to discuss options.
- Assist in completion of applications for Federal, State and Local programs.
- Attend doctor appointments with the patient to actually hear what each doctor is recommending, pass this information on to the family, and help in keeping each doctor informed about what other doctors are recommending.
- Refer to appropriate professionals for additional services, if needed.
- Follow-up with the patient, family, and service providers to monitor progress.
- Collaborate with everyone involved to decide the best course of action to be taken.

Our Three Levels of Service:
- Office consultation. We have found many clients just need to sit down with a professional, discuss their situation and find direction. We will review your circumstances, discuss your concerns and needs, suggest options and provide resources for you to explore on your own. If you decide you would like us to help you on an ongoing basis we can incorporate the findings into our Plan of Care and arrange for our continued Care Management services.
- Comprehensive In-home Assessment. We can come to the home and perform a comprehensive assessment of your loved one’s physical environment and safety, mental health, cognitive and functional status, support systems or services in place. We will review our findings with you, make recommendations for a Plan of Care and implement, monitor and re-evaluate the Plan as needed. We refer to trusted professionals for specific services that may be needed.
- Ongoing Care Management. We will provide our professional Care Management services to you or your loved one on a continual basis. We will provide regular visits, monitor care, link with other professionals involved in the case, coordinate care with other providers and keep you updated about all aspects of your loved one’s care.
